Fisher had a 30-foot putt for eagle to win the tournament at the 18th in regulation play of the 54-hole event, but only managed a birdie to tie with Botes at 10-under par for a 54-hole aggregate of 206.
Moments earlier, Botes had missed a 5-foot birdie putt on the same green which would have moved him to 11-under and the title.
Both played the first two play-off holes to par. Botes let Fisher off the hook, first when he at first missed a 5-foot at the first turn, then a 10-foot putt for birdie with Fisher putting for par at the second turn.
Fisher took full advantage of his lifeline in the third. His tee shot found the left hand rough behind a few saplings and with no clear shot the 29-year-old executed a terrific hook to land five foot from the pin.
"It was a great shot," said the Modderfontein Golf Club professional. "I had 191 metres to the flag, into the wind, and I hit a 6-iron."
Botes' second found the right greenside bunker. He, too, played an exquisite shot out if the trap to within 5- feet from the cup, but Fisher was not about to let a third chance slip away and duly drained his eagle putt.
Earlier in his round, Fisher was 11-under through eight holes but a disastrous triple-bogey on the par-3 11th quashed his momentum. "I came over the top of the ball and landed in a hazard," explained Fisher. "I then duffed my chip shot and missed a short putt. I let a lot of guys in there with that score.
"I told myself to be patient and got back nicely with a birdie on the 14th, missed a short putt for birdie on the 17th and then the birdie on the 18th to force the playoff."
Although Fisher played under par for all three rounds (68, 67 and 71), he admitted to not being on top of his game. "I didn't come here with much confidence," he said. "I did not feel comfortable with my irons but I have been working hard with my coach on that. This week my driving was good, my iron play was suspect and my putting was half decent.
"I played the par-fives very well and that's what won it for me."
This third Sunshine Tour victory bodes well for Fisher, who sets off for the Second Stage of the European Tour School at the end of the month, having recently advanced through the First Stage Qualifier.
